Requesting Services and Accommodations
Students: It is your responsibility after registering for classes each semester to submit this completed form to the DSS office in order to have your Student Accommodation Notice sent to instructors.

Requesting Alternative Format Textbooks
Students: It is your responsibility after registering for classes each semester to submit the Textbook Alternative Format Request Form completed and your proof of purchase to the DSS office in order to receive your textbooks in an alternative format.

Requesting an Interpreter
To request communication access for a Wake Tech event, submit a Sign Language Interpreter/Cued Language Transliterator Request form at least three (3) business days in advance of the event.
Requesting Study Strategy Sessions
For students, registered with Disability Support Services, one-on-one Study Strategy Sessions are available on a first-come, first-serve basis in areas where help is most needed: time management, study skills, organization, curriculum subjects, or test preparations with a Learning Specialist.

Testing
To request testing for South, North, and Online, please access the Permit to Schedule Test Form at least 48 business hours in advance and no sooner than 3 weeks in advance. Please be advised that tests are only accepted between 8:00 a.m.-5:00 p.m., Monday-Friday for fall and spring semester. During the summer, tests are accepted Monday-Thursday between 7:30 a.m. - 6:00 p.m.
To request testing for Perry Health Science, please access the PHS Permit to Schedule Test Form (login required) at least 2 weeks in advance of the test date. Please be advised that tests are only accepted between 8:00 a.m.-5:00 p.m., Monday-Friday for fall and spring semester.
To request testing for the RTP campus, please email [email protected] at least 5 business days in advance. Please be advised that tests are only accepted between 8:00 a.m. - 5:00 p.m., Monday-Friday for fall and spring semester. During the summer, tests are accepted Monday-Thursday between 7:30 a.m. - 6:00 p.m.
